Bridges to Care

Bridges to Care links uninsured residents of Davidson County to a network of some 35 primary health, dental, mental health and substance abuse clinics that serve patients based on their ability to pay. The BTC provider network provides an array of primary care services to the uninsured, which include disease management, treatment of acute episodic conditions, and access to pharmacy services, specialty care, and transportation to medical appointments.

Enrollment Requirements

  • Must be uninsured and live in Davidson County
  • Must complete the application and sign a release of information statement
  • Provide proof of income
  • Provide proof of Davidson County residency
  • Complete interview process with a BTC Care Coordinator
  • Choose a primary care clinic with the assistance of a Care Coordinator

Enrollment Locations, 8:00 to 4:30, Monday through Friday

  • Woodbine Public Health Center - 224 Oriel Avenue; 485-3821
  • Catholic Charities of Tennessee – 10 South 6th Street; 760-2799
  • East Public Health Center – 1015 East Trinity Lane; 862-7916

Once enrolled the patient is assigned to a primary care clinic. Information about the location, services, hours of operation, and charges of the clinic is provided.

Enrollees are also provided information about how to obtain prescription medications for a nominal co-payment either at their medical home or at the two Bridges to Care pharmacies.

Those that have no transportation are provided information about how to schedule transportation to a health care provider that will be paid by Bridges to Care.
