Environmental Engineering Services - Final SoilsItems I, II, III, IV, and V must be submitted to the Metro Health Department, Environmental Engineering Services, before your request can be placed on the active waiting list. As always, please be sure to indicate the map and parcel number of the property you are concerned with. I. Fees Click here for Engineering Services Fee Schedule. Make checks payable to the METRO PUBLIC HEALTH DEPARTMENT. II. Right Of Entry (if not previously submitted) The Right Of Entry form must be completed, signed by the current owner and notarized. We need the notarized original for our file. III. Codes Subdivision Form The Codes Subdivision Form must be completed, signed by a Metro Codes Representative, and returned if not previously submitted. Click here for more information on the Metro Codes Administration. IV. Site Preparation A. Boundaries and Corners Boundary lines and property corners are to be staked and flagged. Access to property is to be flagged and labeled at the road with a large sign stating the map and parcel number and requestor's name. B. Grid Staking The grid must be set by a registered land surveyor. One or more sides of the grid staked area must be on a property line. Alternatively, at least two (2) corners of the grid staked area must be located with shown calls from two (2) surveyed points on the property boundary. Stakes are to be set at 50' intervals, clearly visible, wooden, at least 3' tall, numbered on both sides and flagged or colored with fluorescent paint. Wire flags are not acceptable. C. House Site Proposed house site(s) are to be corner staked and flagged. D. Ground Conditions Grid staked areas are to be mowed or bush hogged. In wooded areas grid lines are to be adequately cut in both directions, permitting walking from stake to stake. The above conditions must be maintained until the septic system has been designed. V. Final Plan (Boundary Survey with Grid Area) A. The final plan must be completed by a registered land surveyor. B. The final plan is to be submitted on a 1" - 100' scale boundary survey. We need 3 copies of the survey. Photocopies are not acceptable. C. Boundary line and corner stakes are to be shown on final plans as staked and labeled in the field. D. Grid points are to be shown to scale as staked and labeled in the field. Also, a 1:1000 degree of accuracy statement pertaining to the grid placement by the surveyor is required.
