Items I, II, IV, and the verification of III must be
submitted to the Metro Public Health Department, Environmental Engineering
Services, before your request can be placed on the active waiting list. As
always, please be sure to indicate the map and parcel number of the property you
are concerned with.

II. Right Of Entry
The Right Of Entry form must be completed, signed by the
current owner, and notarized. We need the notarized original for our file.
III. Site Preparation
A. Boundaries
Tract - flag corners and
property lines of area to be analyzed so that the boundary lines are obvious
Subdivision - flag proposed lot
lines and exterior boundary lines so that they are obvious
B. House Site
Proposed house site(s) are to
be corner staked and flagged.
C. Ground Conditions
Reasonable access leading to
the site is to be maintained and marked. A large sign stating the Map & Parcel
number of the property and the requestor's name will be required at the road.
Mowing or bush hogging will be
required where thick vegetation prevents walking. Any area too vegetated will
be excluded from the analysis!
IV. Preliminary Plan
A. The preliminary plan must be shown on a boundary
survey or enlarged tax map. Property maps can be obtained from the
Metro Planning Commission.
B. Boundaries are to be shown on the plan as they are
staked in the field. State the color of the flagging used (see III., A., above).
C. Proposed house site(s) are to be shown
with labeled distances from a marked boundary. State the color of the flagging
used (see III., B., above).
