Occupational Health and Wellness Center

The Occupational Health and Wellness Center is now located at the Lentz Public Health Building, Room 104, 311 23rd Avenue North. This center provides services for metro employees and metro agencies. The work team consists of a physician, nurse care practitioner, nurses and clerical support. Some of the services rendered include pre-employment physicals, annual physicals, CDL examinations, return to work reviews and special examinations as requested by the metro agency. Services may be obtained by forwarding a written request to the Civil Service Medical Examiner who is the physician in the center.

The Center has the ability to provide Health Risk Appraisal printouts and reviews on an individual basis with aggregate data completed for the entire agency. This service is available upon request from the agency. Each individual will receive health recommendations with educational information based on their Health Risk Appraisal results. The agency will receive an aggregate report of the overall health status of their employees.
 
The Center is a program of the Metro Public Health Department within the line of business of Prevention and Wellness. The mission of the Metro Public Health Department is to provide health protection, promotion, and information products to everyone in Nashville so they can enjoy healthy living free from disease, injury, and disability. The Occupational Health and Wellness Center is committed to upholding the mission of the Metro Public Health Department for Metro employees.
